Welcome to Tissue Viability

The Tissue Viability Service provides a holistic assessment for extensive and non-healing wounds. The team will provide education, support and intervention where required for both patients and healthcare professionals who are managing these wounds.

This support will be provided in the following manner:

• Telephone advisory service
• Virtual reviews via wound photography
• Face to face visit if deemed necessary following triage assessment.

Please note that the tissue viability service will not accept diabetic patients with foot wounds. These patients must be referred to podiatry.

Where is the service provided?

The service is provided in your own home by the community nursing team.

How to access the service

To access the tissue viability service, a referral is required from a health professional who has been managing the wound.
