Welcome to Community Rehab

The Community Rehabilitation Service is a short-term service offering a comprehensive assessment of your current ability and your rehabilitation needs. This is followed by the setting of individualised goals and a treatment plan.

The services include: 

   • Physiotherapy
   • Occupational therapy.

The aim of rehabilitation is to support you to improve your ability to perform activities of daily life and provide a level of independence by using self-management techniques. This may include an exercise programme, the provision of equipment or advice and signposting to support you in managing your own condition.

Where is the service provided?

Community Rehabilitation if provided for service users in their place of residence. 

How to access the service

Referrals are accepted from a range of health professionals (including GPs, Nurses, Hospital Therapists and Doctors) and Adult Social Care, via the Community Gateway.
