Our specialist team of dietitians helps people to make educated choices about food and nutrition. It uses a holistic approach to assess, diagnose and treat diet and nutrition problems.
Working with other community services such as speech and language therapy, community nursing, community matrons and GPs, once referred the team can provide support for those:
- Who have unexplained or unintentional weight loss
- On oral nutritional supplements on prescription
- With gastroenterology conditions, i.e. IBS, coeliac disease etc
- With neurological conditions, i.e. motor neurone disease, Huntington’s disease, stroke patients, Parkinson’s disease, dementia etc
- Cancer patients no longer undergoing acute treatment, i.e. radiotherapy or chemotherapy
- Requiring a change in nutritional intake to improve their health
- Home enteral feeding patients
- Requiring a change in nutritional intake to improve their health.

Where is the service provided?
The team supports people in the community hospitals, out-patient clinics, care homes and in their own homes

How to access the service
Referrals are accepted from all medical, nursing, dietetic and therapy clinicians via the Community Gateway using the appropriate referral form.
Care homes can refer patients directly via the Community Gateway.
Patients with diabetes should be referred to NEEDS .
