Welcome to Homecare and Support Service

Provided by Swan Homecare, this service is there to help people at a time of crisis.  It could be providing extra support in the home due to an illness or accident, without which the person might have to be admitted to hospital.

Swan Homecare will visit the service user within four hours (from original referral if received within working hours) to assess and agree the level and type of care needed, which can be for a maximum of six days. It also provides a handyman service to supply equipment if needed.

The aim of the service is to enable people to return to previous levels of independence, or if ongoing care is needed, to onward refer the person to social care.

 

Where is the service provided?

The service is provided in the service user's own home.

How to access the service

Service users can be referred via their GP, the ambulance service or other community services such as community nursing.

The homecare service is available seven days a week, 52 weeks a year, between 7am and 10pm.

The handyman service is available 9am to 5pm, Monday to Friday.
