More about the One Stop Assessment and Diagnostic Clinic
In this clinic an ultrasound scan of the heart is performed to check the hearts function. The results of the test are reviewed by either a GP with special interest in cardiology, or an advanced nurse practitioner who is highly knowledgeable in cardiology.
If the test confirms a diagnosis of heart failure, you will be able to discuss your diagnosis with a suitable clinician, who will provide you with information and support regarding the treatments available to you and their benefits.
Questions from you and your family or carer are welcomed. Our team will support you to monitor your symptoms and agree your treatment and self-management plans going forward.
Our team involve your GP and hospital consultants if required in decisions about your treatment plan.
If heart failure is not diagnosed, any other possible differential diagnosis will be discussed, and you will be referred back to your GP for further management.
This is a one stop clinic where tests to check the hearts function are performed. The results of the tests are reviewed by a variety of professionals, such as a GP with a specialist interest in Cardiology, a nurse with advanced knowledge and skills in cardiology as well as Consultant input.

Referrals are accepted from GP/practice nurses via the Community Gateway.
With a NT- proBNP of >400pg/ml.
Patient self-referrals are not accepted
